c. Appointment Booking: Patients must provide accurate personal and medical information during the appointment booking process.
d. Cancellations and Rescheduling: Patients are required to provide advance notice for appointment cancellations or rescheduling. Failure to do so may result in fees or limitations on future bookings.
2. Beauty and Aesthetic Services:
a. Consultation and Consent: Prior to any beauty or aesthetic procedure, a consultation will be conducted to assess the patient's suitability and obtain informed consent. Risks, benefits, and alternatives will be explained.
b. Results and Expectations: The clinic provides information about the expected results of beauty and aesthetic treatments. However, individual results may vary, and the clinic does not guarantee specific outcomes.
c. Aftercare and Follow-up: Post-treatment instructions and recommendations will be provided to patients to ensure proper healing and minimize complications.
3. General Practitioner (GP) Services:
a. Medical Consultation: GP consultations are provided for general medical concerns, diagnosis, treatment, and referrals as necessary. The GP will exercise professional judgment based on the information provided by the patient.
b. Prescriptions and Medications: Prescriptions are issued at the discretion of the GP, following a thorough evaluation of the patient's condition and medical history. Patients are responsible for adhering to prescribed medication guidelines.
c. Referrals and Specialist Care: The GP may refer patients to specialists or other healthcare professionals when deemed necessary for further evaluation or specialized treatment.
4. Fees and Payments:
a. Fee Schedule: The clinic will provide a clear fee schedule for all services offered. Fees may vary depending on the nature and complexity of the service.
b. Payment Terms: Patients are responsible for paying fees for services rendered at the time of the appointment, unless prior arrangements have been made.
c. Refunds: Refunds for services rendered are subject to the clinic's refund policy, compliant with the Consumer Rights Act 2015 and other applicable laws.
5. Limitation of Liability:
a. The clinic will take reasonable care to provide accurate and up-to-date information and services. However, the clinic will not be held liable for any errors, omissions, or damages resulting from the use of such information or services, except in cases of negligence or intentional misconduct.
b. The clinic will not be held liable for any adverse reactions, complications, or outcomes resulting from beauty and aesthetic treatments or GP services, except in cases of negligence or intentional misconduct.
6. Governing Law and Jurisdiction:
a. These terms and conditions are governed by the laws of England and Wales.
b. Any disputes arising from these terms and conditions will be subject to the exclusive jurisdiction of the courts in England and Wales.
