Support letter from GP Doctor
Service Description
The doctor will write what he or she knows is supported by your medical record, and his or her opinion on what that should be is final. Kindly note that our doctors reserve the right to refuse your request or provide alternative wording. Patients request letters from their GP for a number of reasons and our doctors are frequently asked to provide private or ‘To Whom it May Concern’ letters or reports for third parties. The usual fee for a standard letter is a minimum of £30. Although more complicated letters or reports may attract a higher fee. Examples of appropriate requests: - Fitness to travel or if you are having to cancel a holiday - Confirmation of medical conditions and / or medication (this could be a summary print out) - Letters for mitigating circumstances, school, universities, employers or courts, etc.
Cancellation Policy
To cCancellation and failure-to-attend fees We require at least 24 hours notice for an appointment to be changed or cancelled. We often have patients waiting for an immediate appointment; failure to notify us in good time means other patients miss available appointments. Cancellations also have cost implications for the practice. Although discretion will be used, a charge will be made if you fail to give adequate notice of your cancellation. Recurrent cancellations at short notice: Pre-payment will be required at the time of booking.
